Your Tandem pump is designed to help you manage your diabetes and reduce the burden of multiple daily injections. You are taking an important step towards improving your overall health.
t:slim X2 Insulin Pump with Control-IQ+ Technology
Your t:slim X2 insulin pump from Tandem Diabetes Care uses rapid-acting insulin only, such as NovoLog or Humalog. You will no longer use the longer-acting insulin that you have been taking once or twice a day. On the following pages, we will explain the difference between these two types of insulin.

Basal Insulin Delivery
Basal insulin, often called background insulin, is the slow and ongoing delivery of insulin that keeps your blood glucose stable between meals and during sleep. The long-acting insulin you have taken by injection is replaced with very small doses of rapid-acting insulin delivered on an ongoing basis by your Tandem pump. The basal rate is the amount of rapid-acting insulin delivered during an hour.

Bolus Insulin Delivery
A bolus is a quick dose of rapid-acting insulin you are giving yourself to cover food or elevated blood glucose. A Food Bolus is taken every time you eat a meal or snack. The Correction Bolus is taken to lower or “correct” high glucose.

Bolus Calculator
The bolus calculator helps you to deliver the correct amount of insulin needed for food or high glucose. Just enter the approximate grams of carbohydrate you plan to eat. Your Tandem pump will calculate how much insulin you need.
It will even keep track of insulin remaining in your body from previous boluses. You can bolus in three ways: for carbohydrate only, high glucose only, or for carbohydrate and high glucose together. Your diabetes team can help you find out which method is best for you.

The Tandem pump contains a motor that will slowly deliver the insulin, along with a software program that tells the pump how much insulin to pump. If using Control-IQ+ technology, the pump will automatically adjust insulin delivery based on continuous glucose monitor (CGM)* readings. It can also deliver an AutoBolus (up to one an hour)† to help prevent high glucose values.
*CGM sold separately. †If glucose values are predicted to be above 180 mg/dL, Control-IQ+ technology calculates an AutoBolus using the Personal Profile settings and a target of 110 mg/dL and delivers 60% of that value.
Having the Right Expectations
You are in charge! While the t:slim X2 insulin pump with Control-IQ+ and a continuous glucose monitor (CGM, sold separately) work together to predict glucose and automatically adjust insulin, they cannot “do everything for you.” To keep glucose under control and stay within your range after eating, you need to enter glucose and meal information into the pump to correct glucose highs and give yourself a bolus to cover for the carbohydrate you are eating.
The bolus calculator will help you figure out the right amount of insulin for food or high blood glucose. For best results and to increase sensor time in range, you should try to enter food and glucose information as accurately as possible, and at the right time (for example, bolus by entering carbohydrate or insulin information to bolus up to 15 minutes before the meal, if this is what your diabetes care team has instructed you to do). To stay successful and on top of your blood glucose management, it is important to be involved in your pump therapy and committed to learning and using the system as recommended.
Important Safety Information
RX ONLY. The t:slim X2 pump with interoperable technology (the pump) and Control-IQ+ technology (Control-IQ+) are intended for single patient use. The pump and Control-IQ+ are indicated for use with NovoLog or Humalog U-100 insulin. t:slim X2 insulin pump: The pump is intended for the subcutaneous delivery of insulin, at set and variable rates, for the management of diabetes mellitus in persons requiring insulin. The pump is able to reliably and securely communicate with compatible, digitally connected devices, including automated insulin dosing software, to receive, execute, and confirm commands from these devices. The pump is indicated for use in persons 2 years of age and greater. Control-IQ+ technology: Control-IQ+ technology is intended for use with compatible integrated continuous glucose monitors (iCGM, sold separately) and alternate controller enabled (ACE) pumps to automatically increase, decrease, and suspend delivery of basal insulin based on iCGM readings and predicted glucose values. It can also deliver correction boluses when the glucose value is predicted to exceed a predefined threshold. Control-IQ+ technology is intended for the management of type 1 diabetes mellitus in persons 2 years of age and greater and of type 2 diabetes mellitus in persons 18 years of age and greater.
WARNING: Control-IQ+ technology should not be used in anyone under the age of 2 years old with type 1 diabetes or under the age of 18 years old with type 2 diabetes. It should also not be used in patients who require less than a total daily insulin dose of 5 units of insulin per day or who weigh less than 20 pounds (9 kilograms), as those are the required minimum values needed for Control-IQ+ to operate safely.
Users of the pump and Control-IQ+ must: use the insulin pump, iCGM, and all other system components in accordance with their respective instructions for use. Failure to follow these instructions for use could result in an over delivery or under delivery of insulin. This can cause hypoglycemia (low BG) or hyperglycemia (high BG) events.
