Instructions for Using Fill-In Forms
- Complete all necessary fields using Acrobat Reader.
- Print the form.
- Sign the form.
- Fax the completed form to Micron Consumer Products Group at (208) 363-5560.

Helpful Hints for Using Fill-In Forms
Use the hand tool or scroll bar to navigate the page.
Click within a form field with the hand pointer. It will change to an I-beam pointer for text entry or an arrow pointer for check boxes. If the pointer does not change, the field is not editable.
Press TAB to move to the next field or SHIFT+TAB to move to the previous field after entering text.
Clicking the space bar toggles check marks in boxes.
Press RETURN to move to a new line within a multi-line field.
District of Columbia Certificate of Resale (Form OTR-368)

This certificate is part of each order and remains in effect until revoked in writing.

Seller Must Keep This Certificate - Instructions
This certificate is valid only if it includes the purchaser's District of Columbia Sales and Use Tax Registration Number, is signed by the owner or authorized officer, and is dated.
If purchasing items not qualifying for tax exemption, the seller should charge the appropriate sales tax. Otherwise, the purchaser must report and pay use tax directly to the Office of Tax and Revenue (OTR) using the relevant Sales and Use Tax returns (FR-800A, FR-800M, FR-800Q, FR-800SE, or FR-800V).
Sellers must retain Certificates of Resale to substantiate exemptions during audits.
To use this certificate, purchasers must file DC Form FR-500, Combined Registration Application for Business DC/Taxes/Fees/Assessments, with the Office of Tax and Revenue, 1101 4th St., SW, Washington DC 20024 (202-727-4829).
