ResMed Astral Series Quick Start Guide
This guide provides essential information for the quick setup and operation of the ResMed Astral series device.
The Astral Device
The Astral device is a sophisticated ventilation system. Familiarize yourself with its components before use.
Device Components:
- 1. Adapter Port: Accommodates single limb leak, single limb, or double limb adapters (Astral 150 only).
- 2. Handle: For carrying the device.
- 3. Inspiratory Port: Connects to the patient circuit.
- 4. Ethernet Connector: For service use only.
- 5. USB Connector: For downloading data to ResScan and connecting approved accessories.
- 6. Mini USB Connector: For ResMed Connectivity Module (RCM) or ResMed Connectivity Module Hospital (RCMH).
- 7. DC Power Inlet: For power connection.
- 8. Device On/Off Push Button: Powers the device on or off.
- 9. SpO2 Sensor Connector: For connecting a SpO2 sensor.
- 10. Remote Alarm Connector: A five-pin connector for remote alarms.
- 11. Low Flow Oxygen Input: Accepts oxygen up to 30L/min.
- 12. Air Inlet: Includes a hypoallergenic filter.
Note: Some accessories may not be available in all regions.
The Astral Device Interface
The device features a touch screen interface for control and monitoring.
Interface Elements:
- 1. Touch Screen: The primary control interface.
- 2. Power Source Indicators: Show AC, DC, or internal battery power status.
- 3. Therapy On/Off Indicator: Indicates therapy status.
- Device Ready: Constant green light when the device is on but not ventilating.
- Device Ventilating: Flashing blue light when the device is ventilating (if Ventilation LED is set to 'ON').
- 4. Alarm Mute/Reset Button: Illuminates for alarms and flashes when muted.
- 5. Alarm Bar: Displays alarm priority.
Touch Screen Controls:
- 1. Clinical Mode Access Button: Locks or unlocks clinical mode.
- 2. Manual Breath Button: Allows manual breaths if enabled.
- 3. Information Bar: Displays key information.
- 4. Internal Battery Indicator: Shows battery status.
- 5. Lock Touch Screen Button: Locks or unlocks the touch screen.
- 6. Menu Bar: Accesses various menus.
- 7. Bottom Bar: Contains navigation and action buttons.
- 8. Start/Stop Vent Buttons: Initiate or stop ventilation.
- 9. Main Screen: The primary display.
- 10. Sub Menus: Access detailed settings.
- 11. Pressure Bar: Shows pressure levels.
Patient Circuits
The Astral device supports various patient circuits, including single and double limb configurations. Circuit adapters are used to connect different circuit types.
Circuit Adapters:
- 1. Single Limb Leak: For single limb circuits with intentional leak or mouthpiece circuits.
- 2. Single Limb: For single limb circuits with an integrated expiratory valve.
- 3. Double Limb (Astral 150 only): For double limb circuits or single limb circuits with intentional leak or mouthpiece circuits.
Fitting the Circuit Adapter:
- Place the device on a soft surface to protect the screen.
- Press and hold the eject button, then pull the cover outwards.
- Lift the old adapter out of the socket.
- Insert the new adapter, ensuring it is firmly seated.
- Align the cover with the device and slide it back until it clicks into place.
Setting Up the Circuit
Proper circuit setup is crucial for effective ventilation. Refer to the Astral Clinical Guide for comprehensive details.
Circuit Configurations:
- Single limb circuit with intentional leak or Mouthpiece circuit.
- Single limb circuit with expiratory valve using ResMed proprietary circuit.
- Single limb circuit with expiratory valve using a standard circuit.
- Double limb circuit (Astral 150 only) or single limb circuit with intentional leak and Mouthpiece circuit.
Performing a Learn Circuit:
This function ensures the device is properly calibrated with the connected circuit.
- From the Setup menu, select the Circuit sub-menu.
- Press 'Start' and follow the on-screen prompts.
Note: Do not connect patient interfaces before performing the Learn Circuit. A Results screen will display upon completion. Error codes, if any, can be found in the Clinical Guide.
Powering On the Device
To power on the Astral device, press the green power on/off switch located at the back of the device. The device will perform a system check.
Upon completion, the Patient Home screen and the active program will be displayed. If multiple programs are available, the active one will be highlighted.
Accessing Clinical Mode
Clinical mode provides access to advanced settings and can be accessed from any screen.
- From the patient home screen, press and hold the lock icon ? for 3 seconds.
- Select '20 minutes' or 'Unlimited' to unlock the padlock and access the Main Settings screen.
To Exit Clinical Mode:
- Press the exit icon ?.
- Press 'Confirm' to lock the padlock and return to the Patient Home screen.
Note: If no selection is made within 7 seconds, the device reverts to the previous screen.
Using the Setup Assistant
The Setup Assistant simplifies the process of setting up the device for a new patient.
- Access Clinical mode.
- From the main menu, press the checkmark icon ✔️.
- Select 'Setup Assistant' and press 'Start'.
- A warning message will appear; select 'Continue'.
- Choose the patient type to configure alarm and setting ranges.
- Select the circuit type to be connected.
- Press 'Start' to run the Learn Circuit function.
- Follow on-screen prompts to attach and test the circuit.
- Select the desired ventilation mode. Default settings for the mode will be displayed.
- Review and adjust settings as needed.
- Press 'Start vent.' to begin ventilation.

Navigating the Menus
The Astral device offers various menus for monitoring, setup, alarms, and information.
- Monitors: Displays real-time waveforms (pressure, flow) and numerical monitoring data. Trends show data over the last 30 days.
- Setup: Includes options for Programs (Setup Assistant, Circuit, Settings, USB, Device Config.), Main Settings, Manual Breath, and Sigh Breath.
- Alarms: Manages alarm settings, priorities, and audible alerts. Includes options for alarm mute/reset and viewing alarm history.
- Information: Provides access to device events, settings, system information, and battery status.
*Available in Clinical mode only. **Software upgrades require an authorized service representative or a USB stick with appropriate software. Contact your authorized agent for details.
Enabling Additional Programs
The Astral device comes with one preset program, but additional programs can be enabled.
- Access Clinical mode.
- From the Setup main menu, select 'Programs'.
- Enable additional programs by toggling the slider.
- Programs can be renamed by pressing the 'Rename' button. A list of preset names is available, or a custom name can be entered.
Note: The currently active program is highlighted in orange and cannot be disabled.
Enabling 'Big Buttons' Mode
The 'Big buttons' mode enhances usability for starting/stopping ventilation and alarm mute.
- From the main menu, press the checkmark icon ✔️.
- In the 'Device Config.' menu, select the 'Patient Access' tab.
- Move the 'Big buttons' slider to 'On'.
This feature can be enabled or disabled in either Clinical or Patient mode.
Alarms
The Astral device provides audible and visual alerts for various conditions.
Alarm Indicators:
- 1. Alarm Display: Shows the highest priority active alarm message. Pressing it provides further details. A triangle symbol ⚠️ indicates multiple alarms.
- 2. Active Alarms Screen: Displays all active alarms, automatically shown when an alarm occurs in patient mode.
- 3. Information Menu: Allows viewing of alarm history.
- 4. Alarm Mute/Reset Button: Used to mute audible alerts or reset alarms.
- 5. Alarm Bar: Indicates the priority of the alarm in the Alarm display.
Alarm Priority Levels:
Alarm Priority | Alarm Bar | Audible Alert |

High | Red flashing light | 10 beeps every 5 seconds |
Medium | Yellow flashing light | 3 beeps every 15 seconds |
Low | Yellow steady light | 2 beeps every 25 seconds |
Attaching Accessories
Various accessories can be attached to the Astral device for enhanced functionality.
Attaching a Humidifier:
- Connect air tubing to the device's inspiratory port.
- Connect the other end of the tubing to the humidifier's inlet port.
- Connect the patient circuit to the humidifier's outlet port.
Attaching a Heat Moisture Exchange (HME):
An HME can be used with double limb circuits or single limb circuits with an integrated valve. Place the HME between the patient end of the circuit and the patient interface.
Note: Do not connect patient interfaces prior to performing the Learn Circuit.
Attaching an Antibacterial Filter:
- Fit the antibacterial filter to the device's inspiratory port.
- Connect air tubing to the other side of the filter.
- Perform the Learn Circuit function.
- Attach the patient interface to the free end of the air tubing.
