How to Claim Your Rebate
To claim your rebate, follow these steps:
- Purchase a qualified product between 6/24/2012 and 6/30/2012.
- Visit www.4myrebate.com to claim your rebate.
- Enter the offer code TD-6451.
- Print the rebate application form.
- Mail in all required documents.
- Allow 8-10 weeks for payment, or select the "No Wait Rebate" option for faster service.
You will need internet access, a valid email address, and a printer to complete the online submission.
Proof of Purchase Requirements
Ensure you include the following:
- Signed Rebate Form
- Original UPC Barcode Label
- Copy of Receipt/Invoice or Packing List

Terms & Conditions
This rebate offer is for qualifying end-user purchasers. Distributors and dealers are not eligible. The purchase date on your sales receipt, packing slip, or invoice must fall within the eligibility period. The address on your rebate application must match your billing address.
Limit of one (1) rebate per qualifying offer, per person, billing address, company, household, and receipt/invoice/packing slip during the eligibility period. Any request postmarked or received after the eligibility period will be denied.
Rebate payments of $5.00 or greater will be issued as an American Express® Prepaid Reward Card. Reward Cards are valid at merchants in the United States, Puerto Rico, and U.S. Virgin Islands that accept American Express Cards. A $2.00 monthly service charge applies after the initial six (6) months, but is waived for the first six months.
If your rebate is less than $5.00, you will be paid by check. Checks are void if not cashed within ninety (90) days of issuance and cannot be reissued.
The rebate sponsor and reward vendor are not responsible for lost, misdirected, or postage-due mail. Incomplete or illegible applications will be denied. Photocopies of UPCs are not accepted unless indicated on the rebate form.
