How to Receive Your $10 Rebate

1. Purchase a Hamilton Beach® FlexBrew® coffeemaker (models 49995R, 49999A, 49983, 49983A, 49983P, 49983S) between 3/31/2014 and 6/1/2014.
2. Complete this form (handwriting must be legible for your submission to be processed) OR go to www.ChooseFlexBrew.com to complete your rebate form online and follow the instructions for mailing.
3. Enclose the original or a copy of the sales receipt or packing slip with the purchase price circled.
4. Enclose the original UPC symbol cut from the bottom of product package.
