- The 1.0 Work GPCI floor expired on December 21, 2024. Accordingly, Work GPCI’s for 2025 do not reflect a 1.0 floor. The 1.0 Work GPCI floor was extended by Section 501 of the Further Continuing Appropriations and Other Extensions Act, 2024 [November 16, 2023] through January 19, 2024. The floor was subsequently extended through December 31, 2024, by Section 303 of the Consolidated Appropriations Act, 2024.
- In calendar year 2024 Medicare PFS, CMS retired three California localities:
- Locality 06, Locality 07, and Locality 26. Locality 06 was previously designated for San Mateo County within the San Francisco-Oakland-Berkley Metropolitan Statistical Area (MSA).
- Locality 07 covered Alameda and Contra Costa Counties, also part of the San Francisco Oakland Berkley MSA.
- Locality 26 represented Orange County within the Los Angeles-Long Beach-Anaheim MSA. These changes were implemented to streamline the locality structure in California from 32 to 29. As a result, the San Francisco-Oakland-Berkley MSA is now represented by a single locality number 5, encompassing San Francisco, San Mateo, Alameda, and Contra Costa Counties. Similarly, the Los Angeles-Long Beach-Anaheim MSA is now unified under locality number 18, covering both Los Angeles and Orange Counties.
- Effective January 1, 2024, the Centers for Medicare and Medicaid Services (CMS) finalized a decision to delete HCPCS code G2066, which was used to report technical services associated with using Insertable Cardiac Monitors (ICM). CMS has added technical service (-TC) and professional service (-26) modifiers to CPT‡ code 93298 in place of G2066.
- The AVEIR™ Leadless Pacemaker patient transmitter supports the remote transmission of certain data elements from a patient’s implanted device. However, it does not transmit heart rhythm data and capture functionality that AMA CPT‡ section guidelines rea a physician or other qualified healthcare professional to review and evaluate to bill CPT‡ code 93294. Per AMA CPT‡ section guidelines, Components that must be evaluated in this code include the battery, leads(s), capture and sensing function, heart rhythm, and programmed parameters.
