Sample Submission Requirements
This document outlines the requirements for submitting a sample device for testing by the FCC Laboratory.
Device Eligibility: Please submit a sample only for devices sold in the United States. The sample must be new, unopened, and unlocked (not carrier locked).
Required Information: Provide a list of all relevant information, such as a country code, that is used to make the device operate as expected when connected to a U.S.-based network. Additionally, include any information or instructions required to perform RF exposure and HAC testing in accordance with relevant KDB guidance.
Sample Type: Pursuant to Section 2.943 of the Commission's Rules, please forward a sample of the referenced equipment. If a production sample is unavailable, a prototype identical to the equipment used for generating the FCC Test Report may be submitted.
Submission Deadline: You have 30 days from the date of this letter to supply the requested sample. Failure to comply may result in dismissal of your application pursuant to Section 2.917(c). Extensions will be considered only upon written request with a detailed explanation of extenuating circumstances.
Labeling and Accessories: The sample and all related accessories, including connecting cables used for testing, must be submitted. All samples and accessories must be labeled with the IDENTICAL identification as referenced above.
Shipping Instructions: The sample MUST be shipped with charges PREPAID. To avoid delays, include a copy of this correspondence and the return shipment information with your shipment. Address the shipping container to the sender of this correspondence.

Information to Expedite Testing: To expedite the testing and return of the sample, please submit the following information:
- A list of each piece of equipment submitted, including its FCC ID, model number, and serial number.
- The complete shipping address, including the name, email, and US phone number for the contact person to whom the sample should be returned.
- Indication of whether the equipment should be returned upon completion of testing.
- The preferred carrier for return shipment and the associated account number.
