Toepassing NL De opklapbare revalidatietafels stellen fysiotherapeuten in staat om revalidatiebehandelingen, fysiotherapie en massage gericht op het behandelen en verlichten van het verloop van ziekten en de gevolgen van blessures/handcipas goed, veilig, en effectief uit te voeren. Bij gebruik in artsenpraktijken stellen ze patiënten in staat om de juiste houding aan te nemen voor een effectieve en veilige procedure of onderzoek. Door de constructie van de revalidatietafels heeft de fysiotherapeut gemakkelijke en volledige toegang tot de patiënt van alle kanten, waardoor allerlei behandelingen het gewenste effect opleveren en de geschikte voorwaarden bieden voor de behandeling/het onderzoek. Opgevouwen is de tafel een koffer. 2. Open het slot (5). 3. Kantel de tafelbladen (1). 4. Verwijder de accessoires (8, 9 ,10) uit de tafel. 5. Klap de poten (2) zo ver mogelijk naar buiten. 6. Plaats de tafel op een harde en vlakke ondergrond (Vóór het belasten moet het midden van het tafelblad ongeveer 3 cm omhoog worden gekanteld). 7. Controleer de spanning van het kabelsysteem (4). 8. Stel de juiste tafelhoogte in volgens de tabel 'Hoogteverstelling'. 9. Installeer de hoofdsteun (9) en andere accessoires (8, 10). 10. Het opheffen van het beweegbare deel van de tafel (A, B) geschiedt door: het veiligheids klittenband los te maken, op te tillen tot de gewenste hoogte.* 11. Het neerlaten van het beweegbare deel van de tafel (A, B) gebeurt door het zo ver mogelijk op te heffen en dan naar beneden te laten zakken en vast te zetten met klittenband.* * geldt voor Xena tafel en tafels met Plus optie 1 IV. Verplaats het beweegbare deel naar een gekozen plaats. 3. Duw de pin erin. V. Verwarmd blad* 1. Zet de bladverwarming aan door op de AAN/UIT-knop te drukken op de afstandsbediening (1). 2. Stel de gewenste temperatuur in door op de toetsen onder het FIEAT V pictogram (2) te drukken, van 1 tot 10. De mat warmt op tot maximaal 32° C. 3. Stel de automatische uitschakelfunctie in door op de toetsen onder het AUTO OFF '+' pictogram of (3) te drukken, van 1 tot 10. De cijfers geven het aantal uren weer. Standaard wordt de verwarming na 1 uur uitgeschakeld. Als we bijvoorbeeld 3 instellen, schakelt de verwarming na 3 uur automatisch uit, enz. 4 Heat Auto Off 2 On/Off 3 OPMERKINGEN: Wanneer de bladverwarming automatisch uitschakelt, kunt u deze resetten door nogmaals op de AAN/UIT-knop (1) te drukken. Als de letter 'F' oplicht op het display van de afstandsbediening (4), haal de afstandsbediening dan uit de tafel en het stopcontact, wacht minimaal 20 seconden en steek de stekker weer in. 1 2 * betreft de Therma Top VI. Positionering van de patiënt De patiënt gaat op het tafelblad zitten en neemt een gemakkelijke horizontale positie in. Bovenstaande activiteiten worden uitgevoerd door de masseur die de patiënt de juiste positie op de tafel aanwijst, en na NL afloop van de behandeling helpt de masseur de patiënt om veilig van de tafel af te komen. VII. Opvouwen van de tafel 1. Demonteer de hoofdsteun (9) en andere accessoires (8, 10) (indien aanwezig). 2. Breng de tafel van de werkpositie naar de vouwpositie (zijpositie met rubberen voetjes). Vouw de buitenste poten op en let daarbij op de juiste positie van de kabels en hun bevestigingsdelen. 3. Plaats de accessoires in de tafel, doe de koffer op slot (5) en plaats de koffer in de hoes (indien aanwezig). VIII. Onderhoud, reiniging en desinfectie van de structuur 1. Gelakte onderdelen. Het wordt aanbevolen om bij het reinigen producten te gebruiken die geen schurende middelen bevatten. 2. Hout, multiplex. Houten of multiplex oppervlakken moeten worden gereinigd met een zachte, vochtige doek en droog worden geveegd. Gebruik geen chemische producten. Opgelet! Hout als natuurlijke grondstof kan verschillen hebben in de rangschikking van de ringen en verschillen in kleurschakeringen en natuurlijke inzetstukken, zoals: strepen, stippen. Dergelijke kenmerken vormen geen basis voor klachten. 3. Bekleding. · Vermijd dat de tafelbekleding in contact komt met oliën. Gebruik hiervoor oliebestendige of waterdichte lakens. · De bekleding moet worden gebruikt en opgeslagen uit de buurt van directe vuur- en warmtebronnen. · Stel de bekleding niet bloot aan langdurig zonlicht. · Gebruik geen: poetsmiddelen, was, sprays, sterke reinigingsmiddelen, oplosmiddelhoudende middelen, producten voor het reinigen van natuurlijk en ecologisch leer. · Olie- en crèmevlekken moeten onmiddellijk worden verwijderd met een mild reinigingsmiddel (een oplossing van warm water met grijze zeep) en een zachte doek of spons. · Alledaags vuil moet worden verwijderd met een mild reinigingsmiddel, bij voorkeur een grijze zeepoplossing, met een spons of zachte borstel. Veeg ten slotte de schoongemaakte zone af met een vochtige doek en wrijf het vervolgens droog. · Lokale, sterkere vlekken moeten worden verwijderd met een 25% oplossing van ethylalcohol, en voorzichtig worden ingewreven met een gedrenkt gaasje. Veeg tenslotte de gereinigde zone af met een vochtige doek en wrijf het vervolgens droog. · Desinfecteer indien nodig met middelen die speciaal zijn bestemd voor desinfectie van PVC- en PUbekleding. Droog de bekleding na desinfectie goed af voordat u deze in de hoes plaatst, zodat deze volledig kan drogen. Anders kan de bekleding verkleuren. Opmerking: Voordat u een ander middel dan een mild reinigingsmiddel gebruikt, controleert u het effect op een onopvallende plek en maakt u deze zeer zorgvuldig schoon. IX. Operationele opmerkingen 1. De tafel is bestemd voor gebruik binnenshuis in droge en gesloten ruimten en voor behandelingen van slechts één persoon tegelijk. 2. Het is niet toegestaan om: de tafel met patiënt te verplaatsen; abrupt op de tafel te gaan zitten (springen); op de hoofdsteun, planken en armleuningen te zitten, knielen, steunen. 3. Let bij het opklappen van de tafel op de opstelling van de kabels. Blokkeer de kabels niet. 4. De tafel moet uit de buurt van directe vuur- en warmtebronnen worden gebruikt en opgeslagen. 5. Er zijn geen contra-indicaties om het product te gebruiken, alleen als is voldaan aan de bovenstaande aanbevelingen. 3 6. Elk ernstig incident met een hulpmiddel moet worden gemeld aan de fabrikant en aan de bevoegde autoriteit van de lidstaat waar de gebruiker en/of de patiënt is gevestigd. X. Ten behoeve van dit document: a) ,,Fabrikant" betekent HABYS Vennootschap met beperkte aansprakelijkheid gevestigd in Jaslo, NL Produkcyjna str. 16, 38-200 Jaslo, ingeschreven in het ondernemingsregister bijgehouden door de Districtsrechtbank in Rzeszów 12e Handelsafdeling van het Nationaal Gerechtsregister, met toegewezen KRS-nummer: 0000513317, met een aandelenkapitaal van 10.750.000 pln (in woorden: tien miljoen zevenhonderdvijftig duizend zlotys), volledig volgestort kapitaal, NIP: 6852208438, REGON: 180186290. b) ,,Koper" betekent de finale Koper van de door de Fabrikant gemaakte apparatuur, waaronder ook de consument en de onderneming. c) ,,Distributeur" betekent de entiteit die op basis van het contract met de Fabrikant de Apparatuur verkoopt in het aangegeven gebied. d) ,,Bevoegde Service" betekent uitsluitend de service uitgevoerd door de Fabrikant of een door hem aangewezen service uitgevoerd door zijn Distributeur, e) ,,Apparatuur" betekent het roerend goed gemaakt door de Fabrikant dat het voorwerp uitmaakt van het verkoopcontract en waarop de betreffende garantie van toepassing is. 2. De fabrikant garandeert een goede kwaliteit en foutloze werking van de apparatuur waarvoor de betreffende garantiekaart is uitgereikt voor een periode van: 5 jaar voor structurele elementen en 24 maanden voor beklede elementen. 3. Reparaties onder garantie omvatten niet de werkzaamheden voorzien in de bedieningshandleiding, die de Koper zelf dient uit te voeren (installatie van de Apparatuur, Onderhoud van de Apparatuur, enz.). 4. De aansprakelijkheid uit hoofde van de garantie dekt alleen defecten die het gevolg zijn van oorzaken die inherent zijn aan de Apparatuur. De garantie dekt in het bijzonder geen schade die het gevolg is van het onjuist of niet in overeenstemming met de bedieningshandleiding uitvoeren van het gebruik, de opslag, het onderhoud of het transport van de Apparatuur, evenals kleurverschillen tussen elementen die al dan niet worden blootgesteld aan zonlicht, chemische of schurende middelen, luchtvervuiling of onbedoelde perforaties en schaafplekken, de aanwezigheid van vlekken of kringen van vettige stoffen of kleurstoffen (vetten, crèmes, lotions of oliën), permanente vlekken met kleurstoffen zoals inkten, permanente markers of andere niet-permanente kleurstoffen die worden gebruikt bij de productie van kleding (bijv. kleurstoffen om jeansbroeken te maken). 5. De garantie dekt geen mechanische schade aan de Apparatuur. 6. De Koper verliest de garantierechten in de volgende gevallen: a) de vaststelling door de Fabrikant dat de Apparatur tijdens de garantieperiode buiten de Bevoegde Service is gerepareerd, b) uitvoering van constructiewijzigingen aan de Apparatuur zonder voorafgaande schriftelijke toestemming van de Fabrikant, c) gebruik van de Apparatuur dat niet conform is met het beoogd gebruik, d) gebruik van de Apparatuur nadat constructiefouten werden vastgesteld. 7. Ongeacht het verlies van de garantierechten, zoals hierboven beschreven in punt 6, is de Fabrikant ook niet aansprakelijk voor schade aan eigendommen of aan derden veroorzaakt door het gebruik van de Apparatuur die onder de garantie valt. 8. De garantie sluit de rechten die voortvloeien uit de garantie uit hoofde van gebreken niet uit of beperkt deze niet. Het uitoefenen van de rechten onder de garantie heeft geen invloed op de aansprakelijkheid van de dealer uit hoofde van de garantie. Als de Koper de rechten onder de garantie uitoefent, wordt de termijn voor uitoefening van de rechten onder de garantie opgeschort op de datum waarop de Fabrikant op de hoogte wordt gesteld van het defect. Deze termijn loopt verder vanaf de datum van de weigering van de Fabrikant om de verplichtingen uit hoofde van de garantie na te komen of het niet-effectief verstrijken van de termijn voor de nakoming ervan. 9. De reikwijdte van de garantiebescherming is beperkt tot het grondgebied van het land waar de Apparatuur is verkocht. 5 GARANTIEVOORWAADEN 10. De Fabrikant komt zijn verplichtingen onder de garantie (verwijdering van defecten of schade aan de structuur van de Apparatuur die voortvloeit uit oorzaken die inherent zijn aan de Apparatuur en die tijdens de garantieperiode aan het licht komen) kostenloos na, binnen 14 dagen nadat de Apparatuur aan de Fabrikant of de Bevoegde Service is bezorgd. Als de reparatie van de Apparatuur niet kan worden uitgevoerd binnen de bovengenoemde periode, zal de Fabrikant de Koper hiervan op de hoogte stellen, evenals de redenen voor de vertraging en de verwachte datum van uitvoering van het erkende garantieverzoek aangeven. 11. Het gebruik van de garantierechten vereist de totale (cumulatieve) vervulling van de volgende vier voorwaarden: a) voorlegging van een btw-factuur, ontvangstbewijs of ander aankoopbewijs van de apparatuur door de Koper, alsook fotografisch documentatie van de defecte Apparatuur samen met een korte beschrijving van de redenen voor het indienen van een garantieclaim, b) melding van het defect binnen 14 (veertien) dagen na de ontdekking ervan, c) levering van de Apparatuur in de originele verpakking op kosten van de Fabrikant, naar het adres dat hierboven op de garantiekaart is vermeld of naar het adres van de Bevoegde Service, d) aanduiding van de plaats van levering van de Apparatuur door de Fabrikant na reparatie (beperkt tot het land waar de Apparatuur is gekocht). 12. De garantieperiode wordt verlengd met de duurtijd van de reparatie, gerekend vanaf de datum van levering van de Apparatuur aan de Fabrikant of de Bevoegde Service, tot de datum waarop de Apparatuur is geretourneerd aan de Koper. 13. Het is de verantwoordelijkheid van de Fabrikant of de Bevoegde Service om de reikwijdte en de methode van reparatie van de Apparatuur te bepalen. 14. De garantie is beperkt tot de reparatie van de Apparatuur. De Fabrikant behoudt zich het recht voor om de Apparatuur te vervangen door een niet-defect exemplaar, als de reparatie de prijs van de Apparatuur vermeld op het aankoopbewijs overschrijdt. De Koper heeft, onder de garantie, niet het recht om vervanging van de Apparatuur door een nieuw exemplaar te eisen. De garantie dekt in het bijzonder geen aansprakelijkheid voor verliezen of schade opgelopen door de Koper in verband met het feit dat hij de Apparatuur niet kon gebruiken. 15. De Fabrikant is niet aansprakelijk voor schade aan de Apparatuur veroorzaakt tijdens de uitvoering van de transportservice door de vervoerder totdat deze door de Fabrikant is opgehaald bij de vervoerder. 16. Indien de verzending van de Apparatuur werd besteld door de Fabrikant, is de Koper verplicht om de technische staat van de Apparatuur te controleren bij ontvangst. Als de Koper vaststelt dat het pakket of de Apparatuur daarin is beschadigd, dient de Koper: a) onverwijld, uiterlijk binnen 3 dagen na ontvangst van het pakket, de Fabrikant op de hoogte te stellen van eventuele schade aan de Apparatuur die is opgetreden tijdens het transport, zodat de Fabrikant een klacht kan indienen bij de transportonderneming en gerelateerde claims kan indienen, b) een klachtenrapport, opgesteld tussen de Koper en de transportonderneming, te bezorgen aan de Fabrikant. 17. De garantiebepalingen in dit document zijn de enige en exclusieve garantieregels voor de producten van HABYS. 6 INSTRUCTION MANUAL I. Application Portable rehabilitation tables enable the physiotherapist correct, safe and effective conduct of EN rehabilitation treatment, physiotherapy, physical therapy, massage, aiming at treating or alleviating the course of illnesses and the results of injuries /impairments. Used at the medical and doctor's offices, they allow patients to adopt the correct posture for conducting effective and safe treatment or examination. The construction of the rehabilitation tables enables the physiotherapist easy and complete access to the patient from each side, thanks to which all various treatments produce desired results and ensure proper conditions for the conduct of a treatment/examination. Open the lock (5) and turn the table tops aside (1). 2. Draw out the equipment (8, 9 ,10) from the table inside (if any). 3. Unfold the legs outside (2) maximally extended and put the table on a hard and even foundation. (Prior to loading the centre of the table top should be tilted up approx. 3 cm). In tables with three pairs of legs, inside legs 11 without load should be slightly raised off the ground and touch the ground when they are loaded. 4. Check the tension of the system of cables (4). 5. Set the appropriate table height depending on the table model description of all the activities is present in the chart entitled "Height adjustment". 6. For cleaning products that do not contain abrasives are recommended. 2. Wood, plywood. Surfaces made of wood or plywood should be cleaned by means of soft damp cloth wiping them until dry. To do this, use oil-resistant or waterproof cover sheets. · Upholstery should be used and stored away from direct sources of fire and heat. · Upholstery should not be exposed to long-lasting sunlight. · Do not use: pastes, waxes, sprays, strong detergents, agents containing solvents, cleaning agents for natural leather. · Dirt from oils, creams must be immediately removed with a mild detergent solution (warm water with potassium soap solution) and a soft cloth or sponge. · Daily soiling should be cleaned with a mild detergent, preferably potassium soap solution with a sponge or soft brush. At the end, wipe the cleaned place with a damp cloth and then wipe dry. · Stronger spot soiling should be cleaned using a 25% solution of ethyl alcohol. Wipe gently with soaked gauze swab. The table is intended for use inside the dry and closed rooms and for the treatment of one person only at a time. 2. It is unacceptable to shift the table along with a patient on it. Rapid sitting on the table (jumping on), sitting, kneeling and basing on the headrest, shelves and armrests is forbidden . 3. While folding and closing the table attention should be paid to avoid snapping the cables. 4. The table should be used and stored far away from the direct sources of fire and heat. 5. Only after meeting the above recommendations there are no contraindications to use the product. 6. Each serious incident related to the device should be reported to the manufacturer and the competent authority of the Member State in which the user and/or patient is established. 9 X. For the purposes of the guarantee: a) "Manufacturer" refers to HABYS Spólka z ograniczon odpowiedzialnoci (Limited Liability Company) with its registered seat in Jaslo, ul. Produkcyjna 16, 38-200 Jaslo, entered into the Register of Entrepreneurs kept by the District Court in Rzeszów, XII Commercial Division of the National Court Register, with the assigned KRS number: 0000513317, with the share capital in the amount of PLN 10.750.000 (ten million seven hundred fifty thousand zlotys), the capital has been fully paid up, NIP: 6852208438, REGON: 180186290. b) "Buyer" refers to the final Buyer of the equipment, manufactured by the Manufacturer, including both the consumer and the entrepreneur. c) "Distributor" refers to an entity that, under an agreement with the Manufacturer, sells the Equipment in a designated area. d) "Authorized Service Centre" refers only to a service held by the Manufacturer or a service indicated by the Manufacturer and held by its Distributor, e) "Equipment" refers to a movable item, manufactured by the Manufacturer, being the subject of the sales and to which the said warranty applies. 2. The manufacturer ensures good quality and efficient operation of the equipment for which the warranty card has been issued, for the period of: 5 years for constructional elements and 2 years for upholstered elements. 3. The guarantee repair does not include the activities provided for in the manual, which the Buyer is obliged to perform on his own (installation of the Equipment, maintenance of the Equipment, etc.). 4. Liability under the guarantee covers only defects resulting from causes inherent in the Equipment., In particular, the guarantee does not cover any damage resulting from improper or inconsistent use, storage, maintenance or transport of the Equipment, as well as colour differences between the parts exposed and not exposed to solar radiation, the influence of chemical or abrasive agents, atmospheric pollution or accidental punctures and abrasions, the presence of stains or rings from greasy or dying substances (fats, creams, lotions or oils), permanent staining with dyes such as inks, permanent markers or other nonpermanent dyes used in the production of clothing (e.g. dyes used to make blue jeans). 5. The guarantee does not cover any mechanical damage to the Equipment. 6. The buyer loses the guarantee rights in the event of: a) the Manufacturer states that the Equipment was repaired by entity other than the Authorized Service, b) making any design modifications to the Equipment without the prior written consent of the Manufacturer, c) using the Equipment contrary to its intended use, d) using the Equipment after revealing any defect in it. 7. Regardless of the loss of the guarantee rights, described above, in point 6, the Manufacturer shall not be liable for any damage to property or caused to third parties, which may be caused by the use of the Equipment covered by the guarantee. 8. The guarantee does not exclude or limit the rights resulting from the warranty for defects. Exercising the rights under the guarantee does not affect the seller's liability under the warranty for defects. If the Buyer exercises the rights under the guarantee, the time limit for the exercise of the rights under the warranty for defects is suspended on the date the Manufacturer is notified of the defect. The period runs further from the date of the Manufacturer's refusal to perform the obligations arising from the guarantee or the ineffective expiry of the time for their performance. 9. The scope of the guarantee coverage is limited to the territory of the country in which the Equipment was sold. 10. The Manufacturer shall perform its obligations under the guarantee (removal of defects or damage to the structure of the Equipment arising from reasons inherent in the Equipment, and disclosed during the guarantee period) free of charge within 14 days from the date of delivery of the Equipment to the Manufacturer or the Authorized Service Centre. If the repair of the Equipment cannot be performed within the above-mentioned period, the Manufacturer shall inform the Buyer of the above, and also indicate the reasons for the delay and the expected date of fulfilment of the recognized guarantee request. 11 WARRANTY CONDITIONS 11. The use of the guarantee rights requires the total (cumulative) fulfilment of four conditions: a) presenting a VAT invoice, receipt or other proof of purchase of the equipment to the Manufacturer, photographic documentation of the defective Equipment, together with a brief description of the reasons for submitting a guarantee claim, EN b) reporting the defect within 14 (fourteen) days of its detection, c) delivering the Equipment in original packaging at the expense of the Manufacturer, to the address indicated above in the guarantee card or to the address of the Authorized Service Centre, d) designation of the place where the Manufacturer delivers the equipment after repairing it (limited to the country in which the Equipment was purchased). 12. The guarantee period is extended for the duration of the repair, starting from the date of delivery of the Equipment to the Manufacturer or to the Authorized Service Centre, to the date on which the Equipment was returned to the Buyer. 13. The responsibility of the Manufacturer or the Authorized Service Centre is to define the scope and the method of repairing the Equipment. 14. The guarantee is limited to the repair of the Equipment. The Manufacturer reserves the right to replace the Equipment with a non-defective one, if the repair exceeds the price of the Equipment indicated in the proof of purchase. Under the guarantee, the Buyer shall not be entitled to demand that the Equipment be replaced with a new one. The guarantee, in particular, does not cover liability for losses or damages incurred by the Buyer in connection with the fact that one is not able to use the Equipment. 15. The Manufacturer shall not be liable for any damage to the Equipment caused during the performance of the transport service by the delivery company until it is collected by the Manufacturer from the delivery company. 16. If the shipment of the Equipment was ordered by the Manufacturer, the Buyer is obliged to check the technical condition of the Equipment upon its receipt. If the Buyer finds that the Equipment has been damaged during the transport, the Buyer is obliged to: a) immediately, not later than within 3 days from the date of receipt of the shipment, notify the Manufacturer of any damage to the Equipment that occurred during transport, in order to enable the Manufacturer to submit a complaint to the delivery company and pursue related claims, b) provide the Producer with a complaint report drawn up between the Buyer and the delivery company. 17.
