How to Claim Your Rebate
- Complete rebate form online at https://event.asrock.com/usretailer/x870_oct_3031_ne.24.asp.
- Verify all fields on the rebate application including name, address, serial number etc. are filled in completely and the information is accurate. The address on your rebate application MUST match either the billing or shipping name and address on your invoice or purchase receipt.
- Print out and sign the completed confirmation page. (Your rebate application cannot be processed without your signature).
- Your screenshot of the review page and the name displayed on your review post must match your Bill To Invoice name. (We do not accept anonymous review screenshots).
- Add the following wording to your review: "This review for ASRock review rebate program."
- Cut out the original model name, serial number & UPC/EAN barcode label from the retail color box and mail with a confirmation page & valid receipt/invoice copy to the address below.
- Ensure all UPC/EAN barcodes and other documentation are securely attached.
- For your protection, keep copies of all submitted paperwork. Copies will be required if your submission is lost.
- Rebate requests must be received and postmarked within 30 days of the purchased date.

Terms and Conditions
Offer valid on purchase of specified product from US retailer for end-users only. Dealers, distributors and other resellers are not participating for this offer. This rebate form must be filled out completely and signed with a copy of sales receipt/invoice, original UPC/EAN code with the model and serial number label from the product package. Incomplete forms will be denied. Limit one rebate per product, per household. Rebate requests must be received and postmarked within 30 days of the purchased date. Allow 8-10 weeks after rebate promotion postmark date ends for processing. The rebate is paid in US dollars, for US residents only. The name and address on the rebate form must match the purchase sales/invoice receipt or the rebate will be denied. The rebate check will be voided if not cashed within 3 months of the date on the check. Requests with an invalid or undeliverable mailing address will be denied. ASRock is not responsible for lost, late or misdirected mail. All documents submitted with the rebate form will not be returned; please keep copies of all documents submitted for your records. No open box items. Warning: Fraudulent submission could result in federal prosecution under mail fraud statutes (Title 18, USC sections 1341 & 1342). If you have not received your rebate check within 8 weeks, you can email rebate@asrockamerica.com to check the status of your rebate.
