1. Introduction

2. Components Included

3. Setup and Installation

Follow these steps to properly set up and install the Wheelchair Identification System:

  1. Select a Location: Choose a visible and easily accessible location on the wheelchair frame for the plastic bracket. Ensure it does not interfere with the wheelchair's operation or user comfort.
  2. Attach the Bracket: The plastic bracket is designed for easy installation. Securely attach one of the 10 provided brackets to the chosen location on the wheelchair. The bracket typically clips or slides onto a frame component.

  • Name: Write the full name of the individual using the wheelchair.
  • Room No.: Indicate the room number or specific location where the individual resides.
  • Chair Accessories: List all specific accessories required for the wheelchair, such as "Lap Cushion", "Foot Rests", "Oxygen Tank Holder", "IV Pole", etc. Ensure all necessary items are noted.
  • Total Chair Weight: Record the total weight of the wheelchair, including any permanently attached accessories, if this information is relevant for lifting or transport protocols.

The labels are removable, allowing for updates or changes as needed. Simply peel off an old label and apply a new one with updated information.
