Setup and Installation
- Turn Off Power: Locate the circuit breaker or fuse that supplies power to the outlet box where you intend to install the device. Turn it OFF. Verify power is off using a voltage tester.
- Remove Old Device (if applicable): Carefully remove the existing switch or receptacle from the wall box. Disconnect the wires, noting their connections.
- Prepare Wires: Strip approximately 1/2 inch (12.7 mm) of insulation from each wire end. Ensure wires are straight.
- Identify Terminals: The Leviton 5225 device has the following terminals:
- Green Ground Screw: For the bare copper or green insulated ground wire.
- Silver Screws: For neutral (white) wires.
- Brass Screws: For hot (black) wires.
- Break-off Tabs: Located on the side of the device, these tabs connect the switch and receptacle to the same hot feed. If you need to control the switch and receptacle from separate power sources, carefully break off the tab connecting the brass screws.
- Wire Connections:
Ground Wire: Connect the bare copper or green insulated ground wire from the wall box to the green ground screw on the device. Tighten securely.
Neutral Wire: Connect the white neutral wire from the wall box to one of the silver screws. Tighten securely.
Hot Wire (Power In): Connect the black hot wire (power source) from the wall box to one of the brass screws. Tighten securely.
Load Wire (for Switch): If the switch controls a light fixture or other load, connect the load wire to the remaining brass screw. If the switch and receptacle are on the same circuit, and the break-off tab is intact, the switch will control the receptacle. If the tab is broken, the switch will control a separate load connected to its brass screw, and the receptacle will be constantly powered.
- Mount Device: Carefully fold the wires into the wall box. Secure the device to the wall box using the provided mounting screws.
- Install Wallplate: Attach a standard wallplate (not included) over the device.
- Restore Power: Turn the power back ON at the circuit breaker or fuse box.
- Test: Test the switch and receptacle functionality.

Operating Instructions
The single-pole switch operates by toggling the lever up or down to control the connected load. The 15 Amp receptacle functions as a standard electrical outlet for plugging in devices. If the break-off tab is intact, the switch will control power to the receptacle. If the tab is removed, the switch and receptacle operate independently.
Maintenance
This device requires minimal maintenance. Periodically inspect the device and wallplate for any signs of damage or wear. Clean with a soft, dry cloth. Do not use abrasive cleaners or solvents.
Troubleshooting
- No Power to Receptacle/Switch: Check the circuit breaker or fuse. Ensure all wire connections are secure.
- Switch Not Controlling Receptacle: Verify that the break-off tab between the brass screws is intact if you intend for the switch to control the receptacle.
- Loose Connections: If the device feels loose in the wall box, ensure mounting screws are tightened.
