FIELD OF APPLICATION

Obturation products: posts and screw-posts. The above obturation products are used to restore damaged teeth. One part of the device is fitted in the root of the tooth while the other part is used to hold the restoration in place. The 2 most common obturation products used are: screw-posts and cemented posts. These devices are threadlike extensions of the obturation fitted in a prepared dentinal or canal cavity. The extensions ensure the restoration products are better held in place. They are generally used by dental surgeons or by prosthodontists in their dental offices. As specified in the instructions, obturation products must only be handled by a professional with experience in dental medicine and only for the designated procedure. The user must follow not only current health and safety regulations concerning the prevention of work-related accidents and work protection requirements, but also the instruction manual. The MD preparation and its maintenance must be done exclusively by persons trained in infection prevention, in self-protection, and patient protection.

According to instructions, it is imperative that users:

  • Use non-defective instruments only,
  • Use tools and products for their intended purpose only,
  • protect themselves, the patient and any third party against all dangers,
  • Avoid any contamination by the product.

Any of the following

  • improper usage,
  • Use of accessories or spare parts not approved by Anthogyr,
  • Use of accessories for this MD with other devices,
  • modification or addition made to the MD without prior validation from Anthogyr shall make the warranty null and void and release Anthogyr of any liability or claims. These MDs comply with the EU Council Directive 93/42/CEE amended by Directive 2007/47/CEE.

GENERAL SAFETY INFORMATION

  • Before using the MDs and their accessories (drill–reamer, burrkey), check that they are not damaged.
  • Should you have any doubts regarding the use of the MD, contact your local supplier or the manufacturer.
  • Wear suitable protective clothing, especially gloves, a mask, and protective glasses during cleaning and disinfection. To avoid risk of infection, follow the guidelines concerning hygiene and sterilisation. Risk of injury: ensure that the motor has completely stopped before activating the locking/unlocking system of the rotary instrument and before handling the rotary instrument (drill, reamer-burr).
  • Use a dam to avoid any slipping of the MD or its accessories (drillreamerburr-key) within the mouth.
  • The practitioner is responsible for selecting the size, shape and material for the post.
  • There are risks of interactions between the MD and the local environment.
  • Only the practitioner can assess what type of material has to be used (titanium or stainless steel).
  • To avoid breaking the castable posts in the mouth, never fit them intraorally.
  • Drills/reamers must be replaced every 20 to 40 uses

MD START-UP PROCEDURE

The MD and all its accessories are supplied non-sterile. Prior to use, the MD must be cleaned, decontaminated and sterilised (See § VI). Before the first use and after each subsequent use, MDs accessories (drills, burrs, reamers…) must be cleaned, decontaminated and sterilised (See § VI). If you notice any visible defects (signs of impacts, manufacturing defects, corrosion …) on your MD or any of its accessories, set it aside, do not use but contact your after-sales service.

MD lifecycle
If cared for properly, drill bits and reamers can be used approximately 20 to 40 times, which corresponds to 20-40 sterilisation cycles.

However, these indications are not a warranty because wear may appear prematurely, depending on how the MD is maintained (cleaning and sterilisation).

HYGIENE AND MAINTENANCE

  • The sterilisation of the MD (if made from stainless steel/titanium) must be carried out by well-trained, well-protected staff, and meet with current health and safety regulations. The sterilisation protocol needs to be adapted to infection risk.
  • Wear suitably protective clothing: risk of infection or injury.
  • Only stainless steel/titanium MDs, drills/reamers or keys can be sterilised. Packaging cannot be sterilised.
  • Only use products intended for the maintenance of medico-surgical equipment and compatible with stainless steel (i.e. no chlorinated content). The use of both skin and mucous membrane antiseptics is prohibited. The use of both aldehyde-based and alcohol-based products, as well as that of any other product likely to cause protein fixation is prohibited.
  • Refer to the manufacturer instruction manual for each product used. Comply with the concentrations, exposure durations, and life span of products. Do not mix products and follow instructions for their disposal.

Cleaning and disinfection
Immerse the MDs (stainless steel/titanium only) and their accessories in a disinfecting solution (an ultrasonic bath can be used). Thoroughly rinse1 , and then dry each element (MD and accessories).

Sterilisation

  • Only sterilise MDs or accessories that have been cleaned.
  • After selecting the most appropriate bag sizes, put each MD and /or accessories into an individual sterilisation bag that complies with current standards.
  • Allow enough space between bags and do not overload the autoclave.
  • The MDs and their accessories can be sterilised at 135°C for a minimum of 20 minutes (sterilisation time).
    1. Temperature < 38°C, distilled water is recommended instead of running water if this has too much chlorine (cf. FD98-135 standard)\
  • Ensure that the device is perfectly dry at the end of the cycle.

PLACEMENT PROTOCOLS

Choosing the post
Place the calibrating film on the periapical X-ray. The calibrating film is full scale; be sure to check the scale of the X-ray. Select the obturation product best suited for the case (length–diameter).

Hygiene
All MDs used intra-orally must be cleaned, disinfected, and sterilised (See § VI).

Preparing the canal
Use the Gates drill to eliminate the remaining gutta-percha in the canal. Progressively widen the canal by using drills of increasing diameter. The last drill used should be the same size as the MD to be placed.

  • Ensure that sufficient dentinal wall remains.
  • Use the stops and the ruler to adjust the reaming depth. The stops are fixed on the drill and provide a visual indication of the depth to drill.
  • Check that the colour codes for both the accessories (drills/reamers or keys) and the MDs are the same ones. If necessary, create a base for the head using a countersink matching the diameter of MD to be fitted (same colour code as the drill/reamer or the MD).

Case of posts

  1. Check the physical entirety of the post (manufacturing defects, signs of impact, corrosion …) before fitting it in the mouth.
  2. Fit the dam in the mouth.
  3. Insert the selected metal post into the hole made. Do not use castable posts to make impressions. There is a difference (2/100) between metal posts and castable posts; castable posts are not as thick, leaving space for the sealing cement, thus avoiding too much friction between the metal and the tooth.
  4. Make the impression.
  5. Send the impression to the laboratory with the metal post still inside, together with the associated castable post (same colour code). In the event of divergent roots, include the corresponding lock(s) (stainless steel and castable).
  6. The temporary crown is created by inserting a metal post, identical to the one used for the impression, in the canal. Only place the temporary cement on the edges of the crown and not on the post.
  7. In the laboratory: Cast the master model, insert the castable post in the master model and make the model. It is cast using the conventional method.
  8. In the dental office: Try on the prosthesis and seal it permanently.

Case of screw-posts and posts

  1. Check the physical entirety of the post or the screw-post (manufacturing defects, signs of impact, corrosion …) before fitting in the mouth.
  2. Fit the dam in the mouth.
  3. Apply the sealing cement using a paste-filler (lentulo).
  4. For screw-posts: use a key to fix the MD in place inside the canal. Use a hollow key or, if there is not enough space, a cruciform key. Use the hole in the key and dental floss to make a safeguard system so that the MD cannot get lost inside the mouth.
  5. RVS and TVS posts are self-tapping, they are screwed and sealed in 4 to 5 turns. The excess cement is evacuated via the grooves. In the event of short roots, tapping can be done with the post before preparation for sealing, thus limiting stress to the root and allowing debris to be removed during tapping. When tapping, screw 1 turn, then unscrew 1/3 of a turn, then again screw 1 turn, etc …

AFTER-SALES SERVICE

The drills/reamers must be replaced every 20 -40 uses. The replacement of spare parts is guaranteed for 7 years after the product is discontinued. Please contact your approved distributor. Should the obturation product be faulty, the MD must be returned complete and sterile with proof of sterility. A document detailing the problem and providing the full contact details of the user-practitioner must be included. Anthogyr cannot be held responsible for damages and/or consequences that result or could result from: normal wear and tear, misuse, incorrect cleaning or maintenance, or from non-compliance with instructions regarding use.
