· Submit a separate application for each eligible charging station.
· You must be the owner of the eligible electric vehicle charging station to be eligible for an incentive.
· Eligible charging stations must be purchased in Canada from a Canadian dealer/web site/retailer.
· Submit your application within 3 months of the purchase/lease date of your charging station.

E - Confirmation of Eligibility If you purchased the charger, you must keep the charger for at least 12 months after the purchase date. If you do not meet Program requirements, you must repay the incentive in full. Any incentive received that you are not eligible for is a debt due to the Crown which means the incentive must be repaid in full and be received by the Province of Ontario, before you can receive another incentive.

When you sign and submit this application, you are warranting that you, as the applicant, have read and agree to all the declaration/consent statements below.

1. You are either an individual or a validly existing legal entity with full power to fulfill its obligations under the Program.

a) You will abide by all the terms, conditions, instructions and notices set out in the EVCIP Program Guide.
b) The Province of Ontario may conduct audits and/or investigations to confirm that you have and are meeting all Program requirements.
c) All decisions made by the Province of Ontario relating to the Program, including applicant eligibility and the amount of the incentive is final and binding and cannot be appealed.
d) You will not sell, lease or otherwise dispose of any electric vehicle charging station acquired with an incentive received pursuant to the Program within one (1) year of such incentive having been received.
e) If you do not meet Program requirements, including any eligibility requirement, or no longer comply with the terms and conditions of the Program, the Province of Ontario may recover from the applicant, any incentive payment or part thereof as a debt due to the Crown.
f) The selection, purchase, installation and ownership/maintenance of the electric vehicle charging station listed in the application form are the applicant's sole responsibility and that the provider(s) of same is/are not affiliated with the Province of Ontario, even if the provider(s) is/are included on the list of eligible suppliers and manufacturers maintained by the Province of Ontario.
g) The Province of Ontario makes no representation, warranty, whether expressed or implied, in respect of any such equipment including, without limitation, any electric vehicle charging station, services or measures that the applicant has chosen and applied for a rebate. The applicant agrees that Her Majesty the Queen in right of Ontario, her ministers, agents, appointees and employees shall have no liability concerning the installation, performance, use or fitness of the same for the applicant's purpose.
h) The applicant agrees that any rebate(s) discount(s) and /or incentive(s) offered and/or provided by anyone, including any supplier or manufacturer, distributor, dealer and /or installer, are separate and distinct from this Program. The Province of Ontario will not be responsible for any rebate(s), discount(s) and/or incentive(s) offered and/or provided by any supplier, manufacturer, distributor, dealer and/or installer.
3. You acknowledge that:
a) Submitting an incomplete application will delay processing the application, or may make you ineligible for an EVCIP incentive.
b) You and the charger must comply with all Program requirements to be eligible for an incentive.
c) You may not apply for or receive any other Canadian, federal, provincial, or municipal, or U.S. federal, state or municipal electric vehicle charger incentive for this charger.
d) The Program may be changed or cancelled by the Province of Ontario at any time for any other reason whatsoever, without notice.
e) Any information provided on, with or pursuant to this application, is being collected and used to implement, and monitor and ensure compliance with the Electric Vehicle Charging Incentive Program.
f) The Province of Ontario may conduct audits and/or investigations to ascertain whether the applicant has met the Program requirements.

As part of this Program, your six-digit postal code will be disclosed to your local electricity distribution company for the purposes of planning for potential impacts on the electricity grid. By submitting this application, should it be accepted, you are consenting to this disclosure.

Personal information collected for the Program, established under subsection 118(2) of the Public Transportation and Highway Improvement Act, will be used for the purpose of administering the Program and verifying compliance with Program requirements.
For the purpose of administering the Program, your personal information may be disclosed to other Province of Ontario ministries, including the Ministry of Finance and Ministry of Government and Consumer Services. In addition, for the purpose of verifying compliance with the requirement for a certificate of inspection issued by the Electrical Safety Authority (ESA), your personal information may be disclosed to the ESA. By submitting this application, should it be accepted, you are consenting to this disclosure.
With your permission, the Ministry of Transportation may share your home address with your local electricity distribution company for the purposes of planning for potential impacts on the electricity grid.
