How to Submit Your Claim

Thank you for choosing Azza Products. To submit your claim, please complete all of the following steps:

1. Please digitally type directly into the fillable boxes below, or print clearly and accurately by hand using CAPITAL LETTERS. The information you provide will be used to verify your eligibility for the rebate and to mail your rebate cheque.
2. Cut out the original barcode sticker (model number and UPC/EAN label) from the product box. Submit it with the signed rebate form and invoice copy. Documents must be postmarked within 10 calendar days of the invoice or delivery date, as verified by tracking. * Please refer to the detailed instructions in the Terms and Conditions below, and follow the 2-Step Procedure in the event of a Canada Post strike.
3. For your protection, we strongly recommend keeping copies of all submitted documents in case of lost or damaged mail.

Terms and Conditions

1. Azza Mail-In Rebate is valid for eligible purchases from Newegg.ca during the promotional period and is available to end-users only. Dealers, distributors, resellers, and consultants are not eligible. To qualify, applicants must: Complete the rebate form digitally or print in CAPITAL LETTERS with a signature; Include the original UPC/EAN barcode label from the product packaging; Attach a copy of the invoice; Ensure the rebate is postmarked within 10 calendar days of the invoice or delivery/pickup date (verified by tracking).
2. Limit one rebate per person/household per promotion. Rebates are issued in Canadian dollars to Canadian residents only. The name and address on the rebate form must match the invoice. Rebate cheques must be cashed within 90 days of issuance; they will not be reissued.
3. Invalid, illegible, or undeliverable submissions will be denied. Azza and the MIR Center are not responsible for lost, late, or misdirected mail. Submitted documents will not be returned—retain copies for your records.
4. Fraudulent claims may result in legal action under Canadian and Ontario law. If MIR qualified but rebate check not received within 16 weeks, email AzzaMIRca@gmail.com. Claims not followed up within 6 months will be void.
5. Late Delivery Exception: If delivery delays prevent meeting the postmark deadline, include: A printout of the Order Delivery/Pickup Notice or A Shipment Tracking Page showing carrier name and delivery date. Attach this to your rebate form and ensure it is postmarked within 10 days of receiving the shipment.
6. Postmark Requirement: Mail must be postmarked at a Canada Post office with a visible date stamp. Rebates received without a postmark may be disqualified based on the MIR Center’s received date.
7. Canada Post Strike Procedure: To avoid disqualification during a postal strike, please follow 2-Step Procedure:
   • Step #1: Email a digital copy of the rebate form, invoice, and barcode to AzzaMIRca@gmail.com within 10 days of receiving your order.
   • Step #2: Mail the physical documents (including the original barcode) as soon as postal services resume.
