A smart step-by-step user guide
Introduction
This guide provides step-by-step instructions for using the Medtronic Smart MDI system, which combines the Guardian™ 4 Smart CGM system and the InPen™ Smart insulin pen system. These components are sold separately.

Setting up the Smart MDI System
Ensure you have the following ready:
- A. Guardian™ 4 transmitter and charger
- B. Guardian™ 4 sensor
- C. One Press serter
- D. Guardian™ 4 oval tape
- E. App installed & Bluetooth enabled

Setting up the Guardian™ 4 app
The Guardian™ 4 Continuous Glucose Monitoring (CGM) system sends glucose readings every 5 minutes, 24/7, providing reassurance and helping users stay ahead of their glucose levels.
- Download the app and create a Carelink™ Personal account.
- Follow the on-screen instructions to pair the sensor.
- Insert the sensor (B) and set up the alerts.

Setting up the InPen™ app
The InPen™ app automatically helps track insulin data, administer personalized insulin doses, and alerts users if a dose is missed.
- Download the InPen™ app and create an InPen™ account.
- Pair the InPen™ insulin pen to the app.
- Follow the app tutorial within the app.

Connecting the Guardian™ app with the InPen™ app
The InPen™ system and Guardian™ 4 system are designed to work together seamlessly.
- Connect the two apps by following the final InPen™ app instructions.
- You will be prompted to enter your CareLink™ Personal username and password.
- The Guardian™ app sends glucose data to the InPen™ system app, providing a one-stop, clear view of your diabetes data.
- Navigate to Settings, then Connections, to add available connections, including Medtronic CGM and InPen™ Reports in Carelink™.
- A Care Partner account can be set up to allow up to 5 care partners to be invited and involved in monitoring.

Start sharing reports
You can send your InPen™ insight reports via email through the app. Additionally, you can share your CareLink™ Personal username with your healthcare team to enable automatic data uploads.
Reminders and Notifications
Reminders can help you stay on track with your diabetes management plan.

- Meal Dose Reminder: Notifies if a dose was not taken within the designated time window.
- Long-acting Reminder: Alerts when a long-acting dose is due.
- Check BG Reminder: Provides an alarm 2 hours after a dose and at bedtime to check blood glucose levels.
- Insulin Age (Cartridge Replace Reminder): Notifies when a new cartridge has not been detected within 28 days.
- Low Battery: Indicates the InPen™ is nearing the end of its 1-year life and may need replacement.
- Temperature: Alerts if the InPen™ detects very high (above 37°C) or very low (below 0°C) temperatures, suggesting potential cartridge replacement.
