In order to receive your Visa prepaid card by mail:
1. Purchase select Corsair products between October 01, 2011 and October 31, 2011 at Amazon.com.
2. Go to www.CorsairRebates.com and enter the promotion number CM2011-81.
3. Fill out and print the pre-registration form for this promotion.
4. Cut out and enclose the ORIGINAL UPC code located on the back side of the paper insert or product packaging.
5. Enclose a copy of the sales receipt or packing slip dated between October 01, 2011 and October 31, 2011 indicating your purchase at Amazon.com.
6. Mail all of these items to the address noted on the pre-registration form. Submission must be postmarked within 21 days of purchase in order to qualify. Do NOT use this form for submittal as it is for instructions and information only.
Visa Prepaid Card Offer Period & Eligible Products:
Visa Prepaid Card: This mail-in rebate offer is valid only with purchases between October 01, 2011 and October 31, 2011 on select Corsair products by an end user customer at Amazon.com. Canadian residents will receive their rebate in the form of a check instead of a Visa prepaid card.
Eligible Participants: To qualify for this prepaid card, end user must be 18 years of age or older. Only the actual purchaser of the qualifying product may participate. End user must keep copies of all materials sent. Materials received become the property of Corsair and will not be returned. Please allow 10-12 weeks for receipt of prepaid card. Corsair is not responsible for lost, late or misdirected mail. Use of multiple addresses to obtain multiple Visa prepaid card requests is fraudulent and could result in federal prosecution under the U.S. and Canadian Mail Fraud statutes. Restrictions: Limit one prepaid card per qualifying purchase per unique product. Limit one prepaid card per product line per household. Maximum prepaid card value $230.00 per submission per household. Visa prepaid card issued in U.S. currency. Valid while supplies last for purchases made in the 50 United States and Canada. Promotion is void where prohibited or restricted by law. Submissions must be postmarked within 21 days of purchase date to qualify.
