The address on your rebate application must match the billing address on the receipt, packing slip or invoice. Limit Fifty Six (56) rebate per qualifying rebate offer, per person, billing address, company, household and receipt/invoice/packing slip during the eligibility period, except were prohibited by law. If your rebate payment is $5.00 or greater, you will receive a prepaid American Express® Reward Card issued by American Express Prepaid Card Management Corporation (the "Reward Card").
