The following will guide you t hrough complet ing a referral form in t he provider port al, so you can help an Oscar member visit a specialist t hey need. St ep 1 Start areferral 1. Select t he ?Submit referral?but t on on t he t op right of t he Oscar member?s page. 2. Find t he referring provider wit h t he search bar. If t hat ?s you, ent er your info. 3. The form will not ify you in real- t ime if you?re able t o refer t he member. If you?re not t he assigned PCP, t hat ?s OK! Cont inue t o submit t he referral and make sure t o ask t he member t o swit ch it in t heir account . Not e: You can submit a referral if you are not t he member?s Primary Care Physician (PCP), but st ill fall under t he same Taxpayer ID number (TIN). hioscar.com/ providers/ resources St ep 2 Provide the details 1. Fill in t he specialit y you?re referring t o (the portal will verify if a referral is required). 2. Ent er t he st art dat e and end dat e, number of visit s, or bot h ? it ?s required. 3. The form allows for an opt ional select ion of in- net work specialit y p r ovi d er s. St ep 3 Submit referral 1. Click ?Submit referral,?which will prompt an opport unit y t o conf irm all t he det ails. 2. Hit ?Submit ?once more ? t he complet ed referral will appear under t he Referrals t ab.
